Waiver & Consent: I, the parent or legal guardian, acknowledge that participation in SILAMBU sports ( Basketball & Soccer) programs involves inherent risks and assume all such risks on behalf of my child. I release and hold harmless SILAMBU, its volunteers, coaches, and staff from any liability arising from participation. I authorize emergency medical treatment if needed and understand SILAMBU does not provide medical insurance. I grant permission for SILAMBU to use photos or videos of my child for non-commercial program and community purposes and confirm my child is medically fit and will follow all safety rules.
